Acting for WALDEMAR GŁUSZKO TRUCK TRANSPORT, hereby, pursuant to art. 6 clause 1 lit. a) Regulation (EU) 2016/679 of the European Parliament and of the Council of 27 April 2016 on the protection of individuals with regard to the processing of personal data and on the free movement of such data, and repealing Directive 95/46 / EC (hereinafter: “Regulation on the protection of personal data”), I request your consent for the processing by WALDEMAR GŁUSZKO TRUCK – as the administrator of your personal data for the conclusion and implementation of the contract for the sale of services offered by WALDEMAR GŁUSZKO TRUCK, as well as for marketing purposes.
TRUCK TRANSPORT WALDEMAR GŁUSZKO as the Administrator of your personal data, pursuant to art. 13 of the Regulation on the protection of personal data, informs that:

- TRUCK TRANSPORT WALDEMAR GŁUSZKO processes and will process the following categories of your personal data: invoice data (NIP, company address, company name), name of the person or persons acting on behalf of the Customer, his phone number and business email , details of the person providing the service (first and last name, PESEL, business phone number, ID number).
- The processing of the above-mentioned categories of personal data by TRUCK TRANSPORT WALDEMAR GŁUSZKO is necessary for the Company to properly perform the services that are the subject of the contract concluded with you.
- The processing of the personal data indicated by WALDEMAR GŁUSZKO by TRUCK TRANSPORT is adequate, relevant and limited to what is necessary for the purposes for which they are processed, i.e. to conduct trade negotiations with you aimed at possible signing and subsequent implementation of the contract concluded with you or contracts for one or several services offered by WALDEMAR GŁUSZKO TRUCK TRANSPORT and also for marketing purposes aimed in particular at the re-conclusion and implementation of the contract for the sale of services offered by WALDEMAR GŁUSZKO TRUCK TRANSPORT

- TRUCK TRANSPORT WALDEMAR GŁUSZKO informs that you have the right to access your data and rectify it, delete it (the right to be forgotten), limit its processing, the right to transfer data, the right to object, the right to withdraw consent to their processing at any time without affecting the lawfulness of the processing carried out on the basis of consent before its withdrawal.
- If you consider that the processing of your personal data by WALDEMAR GŁUSZKO TRUCK violates the provisions of the Personal Data Protection Regulation, you have the right to lodge a complaint with the Inspector General for Personal Data Protection, who will be replaced by the President after the entry into force of the Personal Data Protection Regulation.
